Enrollment in a general secondary education institution
1. Enrollment in primary school
1. Application for enrollment of a child in the first grade of an educational institution submitted to the institution by one of the child's parents in person by June 30, 2020. To statements are attached:
1) a copy of the child's birth certificate or a document certifying the identity of the applicant (when submitting a copy, the original of the relevant document is presented);
2) the original or a copy of the medical certificate in the form of primary accounting documentation N 086-1 / o "Certificate of a student of a secondary school on the results of the mandatory preventive medical examination", approved the order of the Ministry of Health of Ukraine of August 16, 2010 N 682 , registered in the Ministry of Justice of Ukraine on September 10, 2010 under N 794/18089;
3) the original or a copy of the relevant document on education (if any).
If available and at the request of one of the parents, when submitting the application for enrollment, a document confirming the place of residence of the child or one of his parents in the service of the educational institution, the details of which are indicated in the application for enrollment.
During July 1-15, applications for enrollment are not accepted, which does not preclude the right of parents to apply for vacancies after July 15.
The presence of a child when submitting an application for enrollment, lottery or enrollment is not required.
2. If as of June 30 the number of submitted applications for enrollment does not exceed the total number of places in the first (first) class (classes), no later than July 1, an order is issued for enrollment of all children.
The list of enrolled students with their last names is published exclusively in the educational institution.
Information on the availability of vacancies is made public.
3. If as of June 30, 2020 the number of submitted applications for enrollment exceeds the total number of places in the first (first) class (classes), enrollment of children is according to the following rules:
1) up to and including July 1, all children whose place of residence on the territory of the educational institution is confirmed, as well as children who are relatives (adopted) brothers and / or sisters of children studying in the institution or children of employees;
2) until July 15, 2020. including vacancies include children who do not live in the service area of the institution, according to the results of the draw;
3) by July 15 inclusive, those children who do not live in the service area of the institution, but have a desire to receive education according to the educational program used by the educational institution and approved by the separate (separate) class (classes) State Education Quality Service of Ukraine. If the number of submitted applications exceeds the total number of places in such a class (classes), enrollment of children is based on the results of the draw.
Children are enrolled in vacancies in the educational institution provided that the necessary documents are submitted by June 15. If the educational institution to which the child can be enrolled in a vacant place and the educational institution in the territory of which the child resides are located within the same locality, the necessary documents shall be accompanied by a certificate of the educational institution in which the child resides. her expulsion from this institution or non-enrollment in it. Such a certificate must be issued by the educational institution in which the child resides within one working day from the date of application by one of the child's parents.
4. Within two working days from the date of enrollment of children, education shall be published in compliance with it Of the Law of Ukraine "On Personal Data Protection" on the information stand of the educational institution, as well as on the official website of the educational institution:
- list of enrolled students with only their last names;
- announcement of the date, time, place and method of the draw;
- information on the number of vacancies and surnames of children applying for vacancies;
- order of the head of the educational institution on the formation of a competition commission consisting of 3 people for the draw.
5. After July 15, 2020. enrollment in vacancies is as follows:
- before the beginning of the school year - children who have the right to priority enrollment; during the academic year - in the order of receipt of applications for enrollment.
6. Enrollment of children in grades 2 - 4 of primary school takes place in vacant places in the order of receipt of applications for enrollment.
Enrollment in grades 5-9
Enrollment in the 5th grade of the institution occurs after the issuance of an order to transfer students to the 4th grade of the educational institution. Children are enrolled in vacancies (if any) in the following order:
- before the beginning of the school year - children who have the right to priority enrollment;
- during the academic year - in the order of receipt of applications for enrollment.
Enrollment in grades 6-9 of the gymnasium for vacant places takes place in the order of receipt of applications for enrollment.
Information on the number of enrolled students and the availability of vacancies is made public.
Enrollment in 10th grade
Enrollment in the 10th grade of an educational institution occurs after the issuance of an order to transfer to it 9th grade students of the institution who did not show an intention to stop studying there and were not expelled or transferred to another educational institution. Information on the number of enrolled students and the availability of vacancies is made public.
The enrollment order is issued within one working day, after which enrollment takes place in vacant places in the order of receipt of applications.
